Range of Services

Chrysalis Care provides a team of committed, skilled and highly trained carers supported by professionals in the field of adult care. We offer a care service which is unique to the Service User and is continually monitored and regularly reviewed. It enables the Service User to remain in their own home and enhance their lifestyle. It enables them to maintain their sense of identity in the community which we will support them to engage with fully.

Our services include:

  • 24 hour Live-in Care: Supporting the service user to live independently in their own home with a full-time carer/companion.
  • Day/Night Care 1-12 hours per day: Meeting the individual needs of the client e.g. Personal care, nursing care, cooking, light housekeeping, shopping and companionship.
  • Home from Hospital rehabilitation: We will provide nursing care in the clients own home carried out by professionally trained and registered nurses, and/or carers to support recovery and well-being.
  • Respite/Short break Care: Creating space within the everyday relationships of the family by providing short episodes of care for adults. (Use of holiday accommodation can be made available) please ask for details.
  • Emergency Care: With short notice we will provide carers to meet the individual need.

The Registered Manager or Care Manager will visit, free of charge the Service User in their own home to discuss their needs and requirements. If they are happy to proceed we will then carry out a full assessment and compile an Individual Care Plan. Once the Service User and any family/ advocate is happy to have the care put into action we start the matching process. Carers profiles are used to offer choice and suitability of Carer. Carers profiles are used to guide the selection of a suitable carer, they will carry out specific requirements treating the Service User with dignity, compassion and kindness. All care will be reviewed at regular intervals and systems are in place to adjust and update Care plans where necessary.

Overview of the process

Our Carers are introduced to the Service User before any services proceed. All of our Carers wear navy blue uniform with our logo embroidered on it and carry an identity card. They will proceed with the activities on the Individual Care Plan: they will also have time to find out how the Service User is feeling and adjust the service to suit their wishes. The Carer will always complete a daily log, recording the episode of care. They will proceed with the activities on your Individual Care Plan; they will also have the time to find out how you’re feeling and adjust the service to suit your wishes. They will always complete a daily log for you and family members to look at where agreed.

Dignity in care is of paramount importance to us, all of our Carers are respectful and compassionate. They carry out person-centred care that responds to the needs of the Service User they are working with.
